Erasmus-vaihtoon lähtevät opiskelijat täyttävät alla olevan Learning Agreement -lomakkeen jo vaihtoa suunnitellessaan. Lomaketta päivitetään vaihdon eri vaiheissa, esim. jos suunnitellut opinnot muuttuvat vaihtokohteessa. Tallenna lomake itsellesi ja aina muutoksia tehtyäsi myös Oulun yliopiston vaihtotietokanta SoleMOVEen oman hakemuksesi After exchange -välilehdelle. Oman kv-koordinaattorisi tiedot laitetaan kohtaan "Responsible person at the Sending Institution". Hän täyttää myös vastaanottavan korkeakoulun (Host University) tiedot ja neuvoo oikeat oppinainekoodit (Field of education).
Outgoing Erasmus students must fill in the Learning Agreement form below already when planning the exchange. Student must use the same Learning Agreement form also during and after exchange (i.e. in case there are changes to the originally planned studies). Please remember to save the form for yourself and to your SoleMOVE application's After exchange tab always when additions or changes have been made. Your own Faculty/Departmental coordinator fills in the information under “Responsible person at the Sending Institution" and acts as the Contact Person. She/he will also help you with the codes for the Field of education.
